To assist clients living in the community to link to necessary medical, behavioral health, housing, social, employment, educational, and other services essential to meeting basic life needs. Sign-on Bonus: $3000 paid out over the first year of satisfactory employment.

Qualifications: - BA/BS in a human services field with a minimum of one-year's experience in a behavioral health setting is required OR AA/AS in a human service field with a minimum of two-years experience in behavioal health setting.
- Must have understanding/knowledge of the behavioral characteristics and psychosocial needs of the target population.
- Strong written and verbal communication skills are necessary to maintain documentation and work effectively with clients, providers, and the healthcare community.
- Skills & Knowledge Behavioral health
- Bi-lingual English and Spanish speaking are a plus.
- Must have own car, a valid driver's license, and a driving record that is satisfactory to our insurance carrier.
- Criminal History Background Check and fingerprinting are required.
