Lucrative referral incentivesResponsibilities:? Provide primary care services within a post-acute care setting, conducting rounds5 days per week? 5 days per week, minimum 5+ hours in building daily with a morning start time3 facilities( 2 in Greenville, NC and 1 in New Bern, NC). 20 ppd.? Provide oversight and mentorship to our clinicians in the greater Greenville,North Carolina, area? Conduct clinical case reviews, share tools, information, and guidelines as theyrelate to quality of care? Represent TeamHealth in engaging, establishing, and maintaining relationshipswithin the community? Provide clear communication with clinicians, facility leaders and staff regardingcare and services for enrollees? Collaborate with leadership to review care coordination initiatives? Develop intervention plans, and share ideas about network management issue? Work alongside pharmacists on medication management? Complete Medicare required visits? Complete acute, follow up, and discharge visits as necessary? Implement an individualized plan of care for patients' goals? Make recommendations on transitional care, DME, home health services, oroutpatient services after short stay? Facilitate and assist leadership in quality improvement initiatives and riskmanagement? Timely response in chart completions and pharmacy recommendations? Clinicians trained or practicing medicine in the following disciplines areencouraged to apply: family (FM), geriatric, primary, internal (IM), adult, postacute (PAC), home health, hospice, palliative and hospital medicine (HM).Qualifications:? MD or DO degree? Board certification? Active and unrestricted South Carolina medical license? Active and unrestricted DEA? Proven leadership skills and a passion for post-acute care? Excellent communication and interpersonal abilities? Needs Primary Care and Leadership ExperienceCompensation and Benefits:Expected compensation is estimated range of $300K-$350K annually with no cap onproductivity income potential.
