Our Mental Health case manager will provide direct hands on, in person support for a variety of individuals while building genuine, therapeutic, and trusting relationships with others while being sensitive to people's experiences and trauma. In this role you will complete required paperwork and documentation and provide referrals and case coordination for individuals receiving services, including mental health, medical, financial, social, educational, vocational, housing, and legal services. Duties are performed under Rule 79 which include documentation, functional assessments, crisis plans, releases, and individual community support plans in schedule and per performance standards. You will interact with the client on a frequent at minimum once a month, or as needs arise. Further responsibility can include development of relationships with county and state staff, family members, employers, and other agencies as we support the person receiving our services. Overtime this position will allow you to grow, and develop your personal skill set and support this growing division for our company. To be considered for this opportunity you will bring your skills and talent, bachelor's or master's degree in human services or related areas or qualify as a Mental Health Practitioner. Individuals with a social work degree must have valid and current Social Work licensure. At least one year of professional or internship experience in the mental health field required. Must have and maintain a valid Minnesota Driver's License, maintain automobile coverage and have access to an automobile to attend meetings in the office and community (Case Managers are reimbursed for mileage.)
