Duties:



  • OnTrack SEES will use the IPS (Individual Placement & Support) model for community-based services, in additional to intensive case management services.



JOB PURPOSE/SUMMARY



Summary of role of team:



  • The role of team, by virtue of the persons they serve, represent a place where the Centers vision is lived out on a day-to-day basis.

  • The severe and persistently mentally ill (SPMI) population truly embodies the poorest of the poor and the sickest of the sick.

  • Therefore, it is the philosophy of recovery services to live up to this vision by reaching out to the SPMI, regardless of what situation they are in or where they are and open a door of stability through mental health treatment.

  • Staff members assist in ensuring that the consumer/natural support is able to access both cost effective and quality services.



Summary of position:



  • OnTrack SEES will use the IPS (Individual Placement & Support) model for community-based services, in additional to intensive case management services.

  • This position will rotate in a 24 hour/7days a week on-call schedule and provide direct coverage Monday thru Friday.

  • Provides information and referral, agency linkage, advocacy, home visits and follow-up services as defined by individualized education/employment goals of program participants.

  • Provides clinically appropriate transition to clients transitioning out of the OnTrack TN program.

  • Provides crisis intervention and emergency services as needed.

  • Utilizes center wide and community services as needed for client benefit.



TYPICAL WORKING CONDITIONS/ENVIRONMENT



  • This position will be housed in the Anderson County Outpatient Clinic and requires both office-based clinical and administrative work and community-based clinical interactions.

  • The SEES Specialist will be responsible for engaging clients, peers, and community stakeholders in a professional and therapeutic manner.

COMPENSATION:



  • Starting salary for this position is approximately $18.42 /hr based on relevant experience and education.



Schedule:



  • This position provides services Monday - Friday during regular business hours and also includes an on-call component during which the clinician would be available to respond to after-hours crises by phone or in person as needed.



Travel:



  • This position requires utilizing a personal dependable vehicle to conduct Center business.

  • Maintaining a dependable vehicle and certified driver status is a condition of employment.



Equipment/Technical Competency:



  • Ability to use a computer and basic office software is required, including but not limited to Office Word, Excel, Outlook and be able to utilize EMR for clinical documentation.



Equipment/Technology:



  • This position requires the use of computer skills with an electronic medical record (EMR), and telehealth/televideo services may be offered as part of the service delivery.

  • Staff will be expected to maintain work with a Center cell phone and computer.



QUALIFICATIONS - OnTrack TN Employment & Ed Specialist



Experience / Knowledge:



  • Must have course work and or experience in the areas of cultural diversity, human development, etiology and treatment of mental illness, alcohol and drug abuse, physical and sexual abuse, suicide, and mental disabilities.

  • Must pass a drug screen and background check.

  • Excellent verbal, written, and computer skills are necessary in order to work effectively and collaboratively with clients, collaborating agencies, and community professionals, as well as maintaining proper documentation.

  • Clinical staff members must possess reliable transportation, a valid driver's license with an F endorsement OR the ability to acquire an F endorsement on driver's license in order to transport clients as needed.

  • Transporting clients in a personal or Center vehicle is an essential function of this position.



Education / License:



  • A Bachelor's degree in a health-related field of counseling, psychology, or social work or must be licensed as a nurse (LPN or RN).

  • LPNs must have fifteen (15) semester hours of behavioral health and one (1) years of behavioral health care experience.

  • Candidates with a Bachelor's degree in rehabilitation, occupational therapy, criminal justice, and education must have fifteen (15) college-level semester hours of coursework in behavioral health and at least one (1) years of work experience in the behavioral health setting working with children.



Physical/Emotional/Social - Skills/Abilities:



  • Outpatient/Community based environment.

  • This position requires utilizing a personal dependable vehicle to conduct Center business.

  • Maintaining a dependable vehicle and certified driver status is a condition of employment.

  • This position requires training and certification in and the ability to adequately implement a variety of verbal de-escalation methods and personal/client safety techniques.

  • Must have mental ability to exercise sound judgment under pressure.

  • An essential job function is a reliable, predictable 40 hours per week job, on site and in the community, with regular attendance.
