- Develop and implement individualized care plans in collaboration with an interdisciplinary team.
- Actively participate in Quality Assurance and Performance Improvement (QAPI) initiatives.
- Maintain compliance with evolving MDS regulations while providing clear communication with residents, families, and team members.
What You Bring Current RN license in good standing. RAC-CT certification required. - Expertise in MDS 3.0, RAI processes, and individualized care planning.
