Your Key Responsibilities
  • Manage the MDS process with precision, ensuring timely and accurate resident assessments.
  • Develop and implement individualized care plans in collaboration with an interdisciplinary team.
  • Actively participate in Quality Assurance and Performance Improvement (QAPI) initiatives.
  • Maintain compliance with evolving MDS regulations while providing clear communication with residents, families, and team members.
What You Bring

  • Current RN license in good standing.

  • RAC-CT certification required.
  • Expertise in MDS 3.0, RAI processes, and individualized care planning.
  • Strong organizational and interpersonal skills, with a commitment to quality care.
