American Hospice and Home Health Services is currently seeking a Full Time, Part Time or Per Visit Physical Therapist to service Stanislaus County and/or San Joaquin County.







Job description:


As the Home Health Physical Therapist, you will provide physical therapy services to patients according to a written physician's plan of care. This will include assessing and evaluating patients and their home environment, assesses for muscle strength, mobility, gait, ROM. As well as participating in the development of the total plan of care Supervises and overseeing the Physical Therapy Assistants according to organization policy.







Our Home Health Care Mission is to provide professional and paraprofessional services to patients in their homes, assisting them to achieve the highest level of potential in their day-to-day activities.

Requirements:


Responsibilities and Duties



  • Ability to perform evaluation in the home and develop a care plan.

  • Help patients with mobility, movement, and help regain their independence.

  • Devise detailed treatment and care plans for each of the patients.

  • Ability to complete all documentation in accordance with policy and procedure.

  • Flexible, as varied daily activities will include phone, email and written communication, meetings with a variety of healthcare professionals, great understanding of hospice rules and regulations

  • Must work closely with team members to provide the optimal care and services.

  • Ability to manage scheduling visits, documentation time, and meetings effectively.

  • Ability to learn an electronic medical records system.



Qualifications and Skills



  • Licensed Physical Therapist in California

  • 2 years' experience as a physical therapist preferred in Home Health

  • Current CPR Certification

  • Ability to drive within 30-80 miles a day

  • Ability to pass background screenings

  • Ability to function autonomously

  • Strong clinical skills in timely medical documentation

  • Effective communication skills

  • Proficient computer skills

  • Reliable transportation and insurance
