Requirements:

• Board Certified - REQUIRED if Board Eligible(ABIM ONLY)
• Active WA license or IMLC(LOQ in hand/must upload during name clears) - REQUIRED
• Completed Endocrinology Fellowship - REQUIRED
• Malpractice - Clean Preferred
• Availability at time of name clear(Required 30 days of availability from providers start date) - REQUIRED
• ACLS/BLS/DEA - REQUIRED
