Description: Madonna Manor, located in Villa Hills Kentucky, is thrilled to announce an exciting opening for an RN MDS Nurse to be a pivotal part of our leadership team. The MDS Coordinator holds a critical position in ensuring our resident assessments are both accurate and optimized for individual care plans. Key responsibilities for this role encompass, but are not limited to: - Conducting thorough resident assessments (MDS) in accordance with all current regulations and guidelines
- Ensuring the MDS process is completed accurately and promptly
- Working closely with our interdisciplinary team to establish personalized care plans for residents
- Monitoring resident progress and documenting responses to the implemented care plans
- Engaging in the Quality Assurance and Performance Improvement (QAPI) initiatives
- Facilitating effective communication between residents, their families, and other healthcare professionals to ensure superior care delivery
- Remaining knowledgeable about the latest MDS regulations and guidelines for compliance
Full time.

Qualifications:
- Must hold a current RN (Registered Nurse) license
- RAC-CT (Resident Assessment Coordinator-Certified) certification is mandatory
- In-depth knowledge of MDS 3.0, RAI process, and care planning methodologies
- Exceptional organizational and communication skills, with strong interpersonal abilities
- Capable of both independent and collaborative work within a team environment
- Demonstrated commitment to delivering high-quality care to our residents
